The Library Survey is made available in Adobe Acrobat format.  If your browser does not already have the Acrobat Reader plug-in that is required to view and print these documents, please click here to access a free copy of the software.  This survey is "fill-in-able."  This means that you can complete a form/permit from your browser, print it, and then either mail or fax it to the Ferndale Public Library.
